Two different things are called a thumbnail
Instagram uses the word for both the small square crop shown in a profile grid and the full cover frame attached to a video. They are not the same object. The grid square is generated for layout and is cropped hard to fit; the cover image follows the post's real aspect ratio and is stored at a far more useful size, commonly around 1080 pixels on the long edge. Resolving a post returns the second, which is almost always the one people actually wanted.
Custom covers are separate files
When publishing a Reel, a creator can either scrub to a frame or upload a purpose-made cover image. If they upload one, that graphic exists as its own file and never appears during playback — you will not find it by stepping through the video frame by frame, because it was never part of the video. This is why extracting a frame from a downloaded Reel sometimes fails to reproduce the cover you saw in the grid.
Why you might want only the cover
A cover is a few hundred kilobytes against a video's several megabytes. For building a reference board, checking which frame a creator chose, or keeping a visual index of posts without storing the footage, the cover is the efficient object. It is also the only part of a video post that is a still image by nature rather than by extraction.
Covers change, quietly
Creators can edit a Reel's cover after publishing without touching the video, and doing so replaces the stored image at the same object. There is no public history of previous covers, so a cover you saw last month may simply no longer exist. If a particular cover matters, save it when you see it.
What the cover does not include
The play button, the view count, the audio attribution and the account handle are interface elements drawn over the image by the app. None of them are in the file. A downloaded cover is the clean image underneath all of that, which is usually a pleasant surprise for anyone who assumed the overlay was baked in.
